Thank you for your partnership with the University of Arizona! 

While we recommend that students select their agency representation during the online application process, we invite you to utilize the below form in the event the student or partner failed to make an agency selection on the online application form. 

Upon review, we will tag the student under your agency name. If there are any discrepancies or concerns, the International Admissions team will reach out for additional information.

Please include any relevant notes, such as when communication began with the student, or questions you have regarding their application at the bottom of the form. 

Please note: This form must be submitted within 30 days of application submission for tagging.
